The Care Act 2014 describes processes the Local Authorities should use when supporting a child/young carer to move successfully from childhood into adulthood.

Transition applies to these 3 groups:

  • Young people with care and support needs who are approaching adulthood;

  • Adult carers of young people with care and support needs who are approaching adulthood; and

  • Young carers who are approaching adulthood.

Transition Work

Transition to adult care can be very difficult to maneuver. Furthermore, changes to the care and support they receive from education, health and care services, or involvement with new agencies such as those who provide support for housing, employment or further education and training. Effective person-centred transition planning is vital to help young people and their families prepare for adulthood.
