1. Referral Form
To get started, the first thing you’ll need to do is fill out a Referral form. You can easily get this form by emailing us or by visiting our contact us page to request one. If you need any assistance while filling out the form, feel free to give us a call, and a member of our admin team will be glad to help you. Once you’ve completed the form, a team member will reach out to discuss your specific needs and the various services we offer at Springcare Support Services. Together, we will agree on a suitable day for a call to explore how we can best support you, which will help both you and Springcare decide if we can provide the necessary services.
2. Initial Assessment
An initial assessment will be conducted next. One of our team members will reach out to you to gather information for this assessment. The details collected will help shape your care plan, which outlines how our staff can utilise their expertise to enhance your health and wellbeing. As with all records related to our Service Users, this assessment will be done with your full awareness and cooperation, and you can request access to these records at any time. Afterward, we will connect you with a suitable worker who lives nearby and will strive to begin your services within a 1-to-2-week timeframe.
3. Care Coordinator
You will be assigned a Care Coordinator. If you have any questions, feel free to talk to your Care Coordinator or reach out to the Registered Manager, who will be more than happy to assist you.
4. Referral Meeting
Before support begins, the final step is to hold a referral meeting. This introductory meeting is designed to verify the information gathered during the assessment, hand over your green Springcare file, and introduce you to your assigned worker.
