1. Referral Form

The first step is to complete a Referral form. This form can be obtained via email or by simply completing the contact us page and requesting a form. If you need help completing a form please call, and one of our admin team members would be more than happy to assist you. A team member will discuss with you your individual requirements, and the range of Services we can provide at Springcare Support Services. A day will then be agreed with you, when to call so that we can find out how we can support you best, which should form the basis for a decision by both you and springcare as to whether the necessary service can be provided.

2. Initial Assessment

An initial assessment will then be completed. One of our office team members will call you, will document an assessment with you. This set of information will inform your care plan, which illustrates and reviews how our staff gain an understanding of how we can use our skills for an overall improvement of your health and wellbeing. In common with all records regarding Service Users, the assessment(s) will be made with your full knowledge and cooperation, and the records will be available to you upon request. We will then match you to a worker suitable to carry out your service, who lives within a good distance and will endeavour to start services as soon as possible within a 1- 2 week window.

3. Care Coordinator

You will then be assigned a Care Coordinator. If you have any questions, please discuss them with your Care Coordinator, or contact the Registered Manager, who will be very happy to answer them.

4. Referral Meeting

This is an introduction meeting, which will be conducted to confirm the information provided at the assessment, provide you with your green Springcare file and introduce you to your worker.
